St. Gabriel Health Clinic, Inc. (SGHC) was formed out of the concept to provide comprehensive primary care services to the underserved and indigent population in Iberville Parish. It was established in 1993, received its IRS nonprofit designation in 1994, and was designated a Federally Qualified Health Center (FQHC) in 2002.

Throughout its year history, the health center has evolved and grown in order to meet the needs of our patients and improve the health status of the community and is proud of its long history of service. 

SGHC’s mission is to provide primary preventive health services to the medically insured, underinsured, and underserved population of Iberville and surrounding areas regardless of the ability to pay and to increase the span of a healthy life, reduce health disparities and achieve access to comprehensive health services. In fulfilling our mission, SGHC’s services target uninsured, undeserved residents living below 200 percent of the Federal Poverty Guidelines.

SGHC’s vision is to provide strategically placed comprehensive sites linked to inpatient and specialty resources created to address unique health needs, SGHC operates seven in-scope sites in southeastern Louisiana on the eastern banks of the Mississippi River, five school-based sites and two community-based sites.

SGHC works in tandem with its community partners and has collaborative partnerships with the Mayor’s offices of the city of St. Gabriel and Baton Rouge, the Louisiana Primary Care Association, the Gardere Initiative, The Louisiana Department of Health, Woman’s Hospital, and other FQHCs and community-based organizations.

Because of the commitment to quality and hard work of its staff and board of directors, SGHC has earned the Health Resources and Services Administration  Health Center Quality Leader award for 2017, 2018 and 2019. SGHC is a proud member of the National Association of Community Health Centers, Louisiana Primary Care Association, and the Association of Clinicians for the Undeserved. SGHC is recognized by the AHA as a Target BP Program Participant.
